You must be FULLY vaccinated against COVID-19 to participate. Fully vaccinated means you’ve had 2 doses of either the Pfizer or Moderna vaccines, or 1 dose of the Johnson & Johnson vaccine.
You must submit documentation (copy of COVID-19 vaccination card) to apply for the COVID-19 vaccine promotion program.
You must be age 18+ and a member of the Lower Elwha community. (See criteria below)
- COVID-19 fully vaccinated Enrolled Lower Elwha Klallam Tribe Members
- COVID-19 fully vaccinated Lower Elwha Klallam Tribe employees
- COVID-19 fully vaccinated first generation Lower Elwha Klallam Tribe descendants (first generation)
- COVID-19 fully vaccinated significant partners of enrolled Lower Elwha Klallam Tribal Members, 1st generation Lower Elwha Klallam Tribe descendants, and Lower Elwha Klallam Tribal Employees.
